I have had the hydraulics on a forklift fail, so that the forks fell onto a rack and would not lift, but I have definitely never seen a forklift fork fly through the air.   Some units do not have the forks 'attached' to the lifting-frame, but merely rest on them, which explains how that fork almost "got' that worker!  😮
