Vertical sync musts be disabled in graphics driver settings.

but anyway im still lagging even with r_picimp 3 and low settings

\seta r_primitives 2

Its a known issue and does cause shite fps etc......other than a clean install or even trying legacy i think all the other suggestions pretty much cover everything.

wasnt \seta r_primitives 2 not just for Atigfx cards ?

tbh yes but i've always had nvidia cards and have needed to use it myself in the past....think at this point anything is worth a shot lol
