I suspect that the wall-hugging ladder part of that roof-access system somehow slides out from the wall.  That would mean that the bottom ladder is somehow lifted slightly above the sidewalk, probably by the rails that it slides on.  Those rails must be anchored in the wall.  To suspend that whole thing from the roof looks improbable.  :D
