When you have ET installed in your Program Files directory, the game can't write files to the etmain directory unless you have ET running as an administrator.

Here's how to do it:
-Open File Explorer/Windows Explorer

-type "%programfiles(x86)%" into your address bar.(if you're on a 64-bit system. For 32-bit OS, omit the (x86))

-Navigate to the "Wolfenstein - Enemy Territory" directory
-Right click on "et.exe", and open the Properties window

-Open the Compatibility tab, and check "Run this program as an administrator"

-Hit Apply and OK.

 

You should be good to go!
